Instructions:

Please read all instructions before you fill out the registration form.

• Your registration fee includes workshops, meals, and entertainment

• Each individual attending, including support staff, must register and pay the $50.00 conference fee.

• Remember to include any special dietary or accessibility accommodations * see registration page*

• Contact the Northeastland Hotel if you wish to stay overnight. The cost to stay at the Northeastland is $75 plus tax per room for up to four people. Say you are with Speaking Up For Us. You must make reservations before Sept. 7, 2006

• SUFU will not be providing hotel registration services. You must call the Northeastland at 768-5321 if you will be using their services.

• A check for the full amount must accompany each registration.

• Attendees' names must be included on payment. Without names we will assume you have made a donation to our conference.
