INSTRUCTIONS

Please read all instructions before you fill out the registration form. You can also download the entire conference brochure here and fill out the registration form included.

• Your registration fees includes the workshops, meals, meetings and entertainment.
• Each individual attending, including support staff, must register and pay.
• Select the conference(s) you want to attend. Send $50.00 check or money order per person, per conference.
• One form must be filled out for EACH attendee.
• Remember to include any special dietary or accessibility accommodations.
• All conferences are 1 day events. SUFU will not be providing hotel accommodations.
• Please fill out the registration form COMPLETELY. Mail form and check or money order by registration deadlines listed on each conference page or brochure to:

Conference Tips and Information

1. Make arrangements early.
2. Plan your transportation.
3. Check registration deadline for the conference you plan to attend.
4. Read registration carefully or ask a friend, family, support for help.
5. Make sure your support knows what you need from them.
6. Have any meds you need for the day with you.
7. If you need support for your day please have support with you.
8. Supports must register too.
9. You may want to bring money for vending machine, cash bar or merchandise (tee-shirt, pen, water bottle).
10. If you have questions call the number of the conference you want to attend.
